Primary FunctionResponsible for the management of pharmacy professionals practicing in ambulatory primary care practices and leading all pharmacy efforts in primary care, value-based care contracts, and population health.ResponsibilitiesOversee efforts to improve cost effectiveness and improve quality of care and evidence-based prescribing through pharmacy interventions, PCP engagement efforts and patient care activities in primary care practices.Perform supervisory responsibilities such as hiring, interviewing, conducting performance evaluations, ensuring staff productivity, providing disciplinary action and recommending salary increases.Make recommendations for workflow changes, prescribing pattern enhancements and improvements in medical practice operations to improve efficiency, adhere to best practices using evidence-based prescribing and adhere to value-based contracting requirements for quality opportunities.Leverage positive relationships with medical providers and medical group leaders to drive operational improvements.Track and report performance of activities through Key Performance Indicators.Recommend areas for education and PCP engagement to the Integra Senior Director of Quality and Primary Care Performance. Collaboratively participate and lead pharmacy topics for the Integra POD meetings and in practice-based efforts in collaboration with practice operations teams.Comply with all federal, state and Hospital regulatory requirements including DEA, RI Board of Pharmacy, Joint Commission and other regulatory bodies.Participate in contracting with local payers to optimize value to the organization, sit at Integra payer quality and leadership meetings to represent efforts of the pharmacy team, and to collaborate with Integra’s leadership team to drive pharmacy initiatives for the ACO.Prepare reports and implement budget. Provide and submit required records to assist and respond to auditors.Work collaboratively with ambulatory pharmacy to improve prescription retention rates.Propose and advocate for innovative pharmacist patient care activities through expanded collaborative practice agreements.Create a welcoming and professional work environment through effective leadership.Perform all other duties as assigned.Job Qualifications And SpecificationsPharmacy degree and a minimum of five years health system pharmacy experience required. Previous experience in a leadership role required and MBA or other leadership degree preferred. RI Pharmacist’s License is also required.
