Patients per day: Clinic volume 35 patients per day, OR volume variable on case load 10-15 casesSupport staff: Clinic has bladder scanner and Urodynamics and UA. 2 MAs and 1 receptionist in clinicCernerLab onsiteX-ray onsite

Rates MUST be submitted at time of name clear - will not be sent over without - Required, no exceptionsBC/BE Urology - RequiredBLS - RequiredFlight, car, fuel, mileage, hotel is a BR0 cap (including taxes)rental must be refueled before being returnedReverse Invoicing - Provider will use Kronos to clock in/out. No paper timesheets will be used.Do NOT reimburse Med Staff FeesNo lodging booked through 3rd party websites, ie expedia, kayak, etcMcLaren is tax exempt and will not reimburse taxes on lodgingMalpractice rates MUST be included in the rateClean Background - Required for quick privileging (provider will be redirected for multiple MP cases or high payouts)MI License in hand- Required at time of clearCOVID Vaccine
