Coverage will be M-F 8a-430p, clinic patients and OR only.Patients per day: Clinic volume 35 patients per day, OR volume variable on case load 10-15 casesSupport staff: Clinic has bladder scanner and Urodynamics and UA. 2 MAs and 1 receptionist in clinicCernerLab onsiteX-ray onsiteRequirement description : Rates MUST be submitted at time of name clear - will not be sent over without - Required, no exceptionsBC/BE Urology - RequiredBLS - RequiredFlight, car, fuel, mileage, hotel is a BR0 cap (including taxes)rental must be refueled before being returned Reverse Invoicing - Provider will use Kronos to clock in/out. No paper timesheets will be used.
