Facilitates various education and awareness workshops on topics appropriate to recovery. Assists clients in recovery and discharge planning.Division/Program Overview:The program sees adults (18+) and older adults.It is a FULLY community-based program - all services happen at a person's home or community.The clients have complex needs - medication, housing, vocational, independent living and mental health.The intensity for services for each client is about 10 hours per month and once we are fully up and running, we'll have a census of about 50 adults.We are preparing to also start an Adult Outpatient Program which this position would oversee as well. Eventually, this position will have all our Adult programs, including IHOT.Learn more about SBHG at:
