Part Time Position

(morning) (20 hours/w)Dept : Elder DepartmentDuties and Responsibilities:Informing participants of and referring participants to available community services.Participating in the completion of comprehensive assessments and plans of care.Writing service coordination notes in the participant’s health record upon service commencement, when significant changes occur, and at least one time every 90 days or more often as necessary to ensure the health record reflects the current clinical needs of the participant.Providing, as appropriate, coordination of the medically related physical, mental, and behavioral service needs of a participant at the time of enrollment, during treatment and care in the Program, or at the time of discharge.Assisting with any personal, social, family, or adjustment problems the participant may experience at the Program.Referring the participant or family to the appropriate community resources if the participant or the participant’s family requires specialized counseling.Coordinating services if a participant needs services from other community agencies, and no agency is acting as coordinator of services for that participant.Assessing, planning, implementing, coordinating, monitoring, and evaluating the services required to meet a participant’s needs.Facilitating activities for the participants, as needed.Assisting and providing backup support for direct services as needed in the Department.Initial entries and updating database software.Performing other related duties as assigned by the director.Qualifications:Be a Social Worker or have a bachelor or associate degree in Health and Human Services or related studies, with some experience in case management, social services, working with children, or adults in a professional capacity, or related areas preferred.Bilingual (Spanish and English), bicultural, or have knowledge about the cultural values and special needs of the Latin American and Caribbean population living in the greater Boston area.Great interpersonal skills with emphasis on both verbal and written abilities to communicate effectively.Ability to work within a team approach environment and self-motivated to work independently.Flexibility and willingness to assist and work with staff and volunteers in other areas to ensure quality of service and accomplish overall goals/objectives.Ability to keep information confidential.Computer knowledge in Microsoft Word, PowerPoint, Excel, and Publisher.Organized, detail-oriented, and ability to handle multiple tasks.Ability to effectively plan, coordinate, and implement work.Ability to facilitate, monitor, and document group events, client feedback, as well as group interaction.Willingness to learn and participate in improving quality of services and efficiency by attending safety trainings, seminars, and other staff development opportunities as needed.LA ALIANZA HISPANA OFFERS GREAT BENEFITSTO APPLY SEND RESUME TOLA ALIANZA HISPANA, INC.1000 Mass Avenue, Suite 101BOSTON, MA 02118
