This is a unique opportunity to take on one of our country’s most challenging healthcare problems and join a fast-growing, dynamic team as we prepare to scale our mission to serve more markets.About this RoleThis is an opportunity for a mission-driven team player who has experience managing home care services to develop and implement home care services for PACE program participants, including the coordination of all Durable Medical Equipment and community-based care. You are a collaborative team member who works as part of the Interdisciplinary Team to create a care plan to help participants meet their goals. This is primarily a center-based role.Expectation for this RoleCoordinates Home Care Services for participants using home care agenciesParticipates as a member of the Interdisciplinary Team (IDT), completing home assessments, care plans, and retrieving information on Home Care ServicesConduct nursing care within LVN Scope of practiceProvides general nursing care in the clinic and eventually, the participant’s homeDocuments in the EMR within set documentation timelineCommunicates with home care employees to understand participant’s condition and reports condition, including reactions to drugs and treatments, to the RN and PhysicianCollaborates with the Transportation Coordinator to ensure accuracy for participant transportationSchedules Home Care appointments and supports the scheduler for other appointment needs, if neededOversees visits completed by external agenciesAttributes of an Ideal Candidate

Passion and mission orientation for serving high-risk seniors and frail older adultsLVN License requiredCPR and First Aid certification (within 60 days of employment)Experience with frail and elderly adultsHome Care ExperienceProof of a valid CA driver’s license, personal transportation, good driving record, and auto insurance as required by State lawLocation
