Staff Physician for PACE ProgramOur Client is seeking a dedicated Staff Physician to join their innovative PACE (Program of All-Inclusive Care for the Elderly) team. In this role, you will provide comprehensive primary care to PACE participants across multiple settings, ensuring that frail and elderly individuals receive holistic, value-based care. As part of an interdisciplinary team, you will support participants’ health journeys through personalized care plans, collaborating with caregivers and managing clinical decisions to enhance their quality of life.Who You AreYou are a compassionate healthcare provider with a passion for working with elderly populations. With at least 2 years of clinical experience and 1 year of experience with frail, elderly patients, you have developed strong interpersonal and collaboration skills. You thrive in a team setting and are adept at managing complex clinical cases, transitions of care, and value-based care models. Known for a collaborative and innovative model of care, Our Client is focused on building a patient-centered approach from the ground up, ensuring that each team member has a meaningful impact on the delivery of care.Your RoleAs a Staff Physician for the PACE program, you will:Provide primary care across settings, including PACE centers, participant homes, virtual appointments, and skilled nursing facilities (SNFs).Collaborate with interdisciplinary teams to create and manage holistic care plans.Manage participant panels and make key clinical decisions, including medication management and referrals.Support participants during hospital stays and transitions of care.Educate caregivers and participants on health management and community resources.Ensure accurate and timely documentation to meet regulatory requirements and contribute to value-based care initiatives.How You QualifyDoctorate Degree from an accredited School of Medicine or Osteopathy.Active, unrestricted MD/DO license in California.Board Certification in Internal Medicine or Family Practice (Geriatrics preferred).State CDS Certification and DEA Registration.Current CPR certification.At least 2 years of clinical experience, with 1 year of experience working with frail, elderly populations.Strong communication and interpersonal skills, with a focus on team collaboration.How You Are SupportedAs a full-time member of Our Client's team, you will receive:Comprehensive benefits, including medical plans (80% premium coverage for employees and 75% for dependents).401(k) with employer match.Paid time off (PTO) and professional development opportunities.Additional benefits, including child care stipends, paid parental leave, and CME reimbursement.Join UsIf you are passionate about working in a truly collaborative, innovative healthcare model and are excited about making an impact, we encourage you to apply.
