The goal of Project Action is to provide incarcerated individuals in Florida Department of Corrections pre and post release services, specifically occupational skills training, work readiness, and case management services pre-release and job placement and navigational case management post-release to help participants transition and reintegrate into their communities. The Case Manager will maintain responsibility for Navigational Case Management (tiered, triage formatted case management services) for participants under Project ACTION. Case Management is a goal-oriented process between the participants and the Case Managers from program enrollment through the follow-up period. Case Management includes administering multiple assessments including Level of Service/ Case Management Inventory, planning, coordinating, monitoring, and resource development.Essential Duties and Responsibilities

Conduct outreach and execute recruitment processes consistent with the program goals and objectives.Conduct participant intakes, eligibility verification and documentation according to funder and Gang Alternative, Inc.’s guidelines.Administer multiple evidence-based assessments.Utilize assessment results to inform the development of the participants’ Individual Development Plan (IDP) that includes the agreed upon education and training goals, career plan, employability skills, etc.Provide guidance and monitor individual participant activities as outlined in the IDPs in order to track progress of individual participants toward attainment of identified goals.Track participant IDP and enter case notes at least bi-weekly.Carry a coaching case load of up to 65 participants continuing for a minimum of one year, including academic/career planning, goal setting, crisis management, and advocacy.Provide case management services, training, academic guidance and mentorship to participants as needed.Accurately track, input data, and maintain comprehensive case files (physical and electronic) for participants.Facilitate group and one-on-one mentoring.Coordinate links to appropriate support services and provide appropriate referrals such as transportation assistance, basic needs and emergency assistance, academic tutoring, counseling, family health support and access to other support services.Track referral outcomes, detailing referral services received and impact on the achievement of IDP goals.Prepare reports, as required, for submission to funder and other partners via Program Manager.Build and maintain key relationships within participants.Collaborate with other organizations and staff engaged in appropriate recruitment activities.Serves as an Ambassador of Gang Alternative, Inc.Implement navigational case management with performance excellence and contract compliance.Qualifications and Competencies

Minimum Bachelor’s Degree in Social Work, Psychology or related degree. Master’s Degree in Social Work preferred.Minimum of three (3) years relevant experience in case management and/or providing training, mentoring, support to disadvantaged populations, including at-risk and justice-involved youths and adults.Demonstrated experience in education and experience developing relationships with service providers.Ability to build and maintain relationships and partnerships; maintain confidentiality, professionalism and maturity in handling sensitive information, participant interaction and community relations.Knowledge of case management principles, objectives, standards, and methods.Knowledge of Miami-Dade County community and government service delivery systems.Knowledge of motivational interviewing concepts and techniques.Strong leadership, organizational and analytical skills.Ability to organize, plan, and manage multiple tasks.Ability to manage time efficiently and meet deadlines.Excellent interpersonal skills.Ability to collaborate and take a team-oriented approach.Ability to work with and engage individuals of diverse backgrounds.Exceptional written and oral communication skills.Must possess proficiency working with Microsoft Office Suite and the ability to successfully navigate through the Internet, Excel, PowerPoint, and other applicable software.Skill to communicate ideas and instructions clearly and concisely.Skill to coordinate with other staff, departments, officials, agencies, organizations, and the public.Other duties as assigned.Note:This position may require independent travel between workstation and other sites.
