*Position Overview:*As a Compassionate Physical Therapist at Advanced Care Home Health, you will have the unique opportunity to make a positive difference in the lives of our patients. You will work closely with a multidisciplinary team to create and implement personalized care plans, fostering an environment of healing and support.
*Responsibilities:** Conduct comprehensive assessments to determine each patient‘s unique needs and goals.* Develop and implement individualized care plans that focus on improving mobility, strength, and overall functional independence.* Provide hands-on therapeutic interventions with empathy and compassion.* Educate patients and their families on exercises and techniques to promote ongoing wellness.* Collaborate with other healthcare professionals to ensure holistic and patient-centered care.* Maintain accurate and timely documentation of patient progress.
*Qualifications:** Licensed Physical Therapist in the state of Oregon.* Proven experience in home health or a strong desire to transition into this setting.* Excellent communication and interpersonal skills.* Compassionate and empathetic approach to patient care.* Ability to work independently and collaboratively within a team.
