OVERVIEW:

Neighborhood Health is a Federally Qualified Health Center (FQHC) that serves a predominantly low-income population in Northern Virginia. Our mission is to improve health and advance health equity in Alexandria, Arlington, and Fairfax County by providing access to high quality primary care regardless of ability to pay. We provide primary medical care, dental care, behavioral health, HIV care, vision care, pharmacy services, and health insurance enrollment to 40,000 predominately low-income patients in multiple clinics across Northern Virginia. Neighborhood Health is accredited by the Joint Commission, certified as a Patient Centered Medical Home, and has been recognized for the quality of care that it provides to its patients. Neighborhood Health’s Ryan White Program serves approximately 400 HIV+ clients, offering primary medical care, medical and non-medical case management, oral health services, mental health services, and medication assistance. This is currently provided through several funding sources: Part A, Part B, and the Minority AIDS Initiative (MAI).

PRIMARY FUNCTIONS:

Serves as organization’s lead for Neighborhood Health’s Ryan White services, ensuring adherence to required program standards. Exhibits excellent organizational, program management, and supervisory skills. Responsible for timely and accurate clinical and qualitative reporting to funders on progress of services. Supervises a team of Ryan White medical and non-medical case managers, and serves as a resource to clinical and administrative staff. Provides support for case management to team members with complex cases. Produces accurate charting and data-entry into multiple databases. Maintains understanding of and compliance with current standards, requirements, and procedures. Demonstrates excellent cross-cultural communication with clients and other members of staff. Also responsible for overseeing Ryan White quality program: responsible for all aspects of planning, developing, reviewing, monitoring, and reporting on RW quality work plan and on the program’s quality improvement projects.

DUTIES/ RESPONSIBILITIES:

PROGRAM OVERSIGHT & MANAGEMENT DUTIES:

Provide overall leadership to the Ryan White program to assure high quality patient care and adherence to current standards.

Represent NH Ryan White program in meetings with funding agencies’ representatives, such as monthly check-ins, quarterly contractors’ meetings, ad hoc consultations, etc.

Submit accurate monthly narrative reports and data submissions to contractors in a timely fashion.

Review and provide accounting team with accurate client usage reports, such as for medical transportation, insurance support, specialty dental care, etc.

Assist department head and accounting team with planning, budgeting, invoicing, and other requests from funding agencies.

Supervise work of Medical Case Managers, Non-Medical Case Managers, QI Assistant, and Oral Health Coordinator to ensure program goals are being met. Recruit to fill any vacant positions, provide orientation and training; review productivity, and complete performance evaluations.

Ensure RW Nurse Case Manager fulfills program quality and productivity goals; collaborate to ensure case management team’s adherence to standards and assist with complex case management cases, as needed.

Work closely with Non-Medical Case Managers to ensure any clients receiving services are currently eligible for services before offering services, and to ensure accuracy in client charts and databases.

Oversee work of Oral Health Coordinator to ensure accurate reporting, adherence to budget, and access to services for eligible NH RW clients and external RW referrals.

Track case management and other service outcomes to ensure high-quality provision of services, and required procedures and documentation undertaken, such as accurate and timely client eligibility, MCM assessments, service plans, provision of core and support services, etc.

Prepare for and serve as key program representative for any Ryan White external/peer audits and site visits. Assist department head with developing a Corrective Action Plan (CAP) or other response, if indicated, and train/monitor MCM/Non-MCM staff on procedures needed to address the findings.

Lead monthly Ryan White staff meetings, preparing agenda and ensuring notes are taken and shared.

Serve as liaison with the Mid-Atlantic AIDS Education & Training Center (MAAETC) for Neighborhood Health’s Practice Transformation activities, trainings and reporting, as applicable.

Maintain up-to-date knowledge of HIV standards of care on a continuing basis and disseminate the knowledge to other members of the team on a timely basis to ensure compliance.

Confer with other local/regional Ryan White programs to prevent duplication of services and other topics.

Perform duties and assignments within established time frames; meet quality and quantity performance measures for the position; demonstrate a level of quality, efficiency, and accuracy.

Maintain patient confidentiality at all times by controlling the information being disclosed to authorized individuals and ensuring compliance with all HIPAA standards.

Serve as liaison and confer regularly with NH’s HIV Prevention and Early Intervention Manager.

QUALITY IMPROVEMENT DUTIES:

Serve as primary staff member responsible for Ryan White Quality Improvement efforts.

Maintain knowledge of current national Performance Measure standards and national/regional outcomes.

With input from other members of the Ryan White team and department head, develop and finalize program’s annual Quality Management Plan, Quality Management Work Plan, and Quality Improvement Projects (QIPs).

Convene quarterly meetings of the RW QI Committee to review progress, share results, develop strategies, and create accountability for all RW quality activities.

Serve as liaison with consumer member(s) of the QI Committee.

Assign other RW staff to participate in quality improvement tasks, support follow-through, and troubleshoot questions/concerns.

Maintain minutes of QI Committee meetings; report on meetings to grant monitors.

Submit agency’s required reports (RSR, Cross-Parts, monthly reports, etc.) in an accurate and timely manner.

Participate in all local and regional quality meetings on behalf of the RW program. Attend regional and/or national-level trainings, as indicated.

Serve as representative from the Ryan White program to Neighborhood Health’s Quality Committee.

QUALIFICATION/EDUCATION and/or EXPERIENCE REQUIREMENTS:

A Master-level degree in health-related field and at least 5 years of program management experience post-graduation. At least 3 years’ experience running a Ryan White or other complex care management program.

Excellent interpersonal communication skills; ability to work effectively with diverse work force and clients.

Strong critical thinking and analytical skills.

Experience with information technology, electronic health records, medical records, culturally diverse populations and care of underserved populations.

Experience with database maintenance and reporting.

Knowledge of local health care environment.

Motivated by change dynamics; high-level ability to manage change.

Travel Requirements:

Occasional travel to other local or regional agencies for meetings and trainings. All work-related required travel outside of northern Virginia is reimbursed at the government rate per mile.
