POSITION SUMMARY

The Data Coordinator will input, gather, record and evaluate CSMD RW Program related

data and manage overall RW Program data management systems (i.e., EHR, CAREWare,

etc.) . Collect and analyze data, interpret and track trends prepare reports and

presentations. Ensure accuracy, integrity, security, and consistency of data, identify and

resolve data-related issues, discrepancies and concerns, and create comprehensive data

report by compiling data from multiple sources.

PRIMARY RESPONSIBILITIES:Supports the mission and vision of CareSouth Medical & Dental; comply with CareSouth's Employee Handbook, Compliance Plan and the Policies and Procedures Manual.Ability and willingness to accept direction from supervisor.Practice a culture of safety to reduce or prevent risk of injury, claims, loss, or liability by utilizing the Risk Management and Infection Control Plan.Perform responsibilities with accuracy in a timely manner with a high-quality standard to yield improved compliance, quality, and patient outcome measures.Report any significant concerns to supervisor in a timely manner.Practice a culture of safety to reduce or prevent risk of injury, claims, loss, or liability in accordance with both internal and external established Risk Management and Quality Control measures.Attend and actively participate in required meetings (i.e., in-services, departmental, QM, Eligibility, Gap in Care, etc.), trainings (i.e., RW Program related, annual CSMD, etc.) and community events. (i.e., national awareness day events, health fairs, etc.).Work cohesively with team members and outside agencies to comply with and meet RW Program requirements as defined by but not limited to CSMD, OPH, BRTGA, HRSA, etc. on mutual data-driven projects.Collect, input, evaluate, sort, record , analyze data for reports, monitor and manage data is designated platforms for CSMD RW Program.Present data reports as needed i.e. excel spreadsheets, PowerPoint presentationsResolve data-related issues and concerns, create accurate and comprehensive data reports, and work with management and/or outside agencies to ensure adequate storage and appropriate access to CSMD RW Program related data.Maintain client financial files (i.e., hardcopy and electronic)for EHE services (i.e., gas cards, food cards, housing and utilities assistance documents with attachments). Including transportation consents, Budget and Needs Assessment Forms with proof of payment.Comply with Louisiana CAREWare Access Network (LACAN) Ryan Whie CAREWare Policies & Procedures.Manage CAREWare Data systems for Ryan White Program to meet planning, grant reporting, quality and performance measurement requirements.Accurately enters required patient information (i.e., visits, STI & HIV labs, pregnancy status, and ARTs) and as needed update demographics (i.e., vital status- deceased or out of jurisdiction, ethnicity, HIV risk factor, and the patient address) into CAREWare daily for RW Providers.Reviews and updates patient demographic information in charts in the EHR and data system.Demonstrates knowledge of basic medical terminology, clinical procedures, testing, etc.Assists with managing patient flow as needed.Exhibits a proven capability of maintaining confidentiality within a clinical setting.Maintains a high level of ethics in managing patient confidentiality and be a strong advocate for persons living with HIV/AIDS, Hepatitis C patients and their families.Arrange travel for eligible RW clients in accordance with established transportation guidelines and funding to and from meetings, health care appointments and to meet social services related needs.Updates daily status in the EHR and data system for preparation of monthly, daily and quarterly reports.Manage and run custom reports from CAREWare (i.e., eligibility, transportation, mental health, EIS, demographics, outpatient ambulatory ,etc.) monthly.Manage and run monthly Performance Measures reports via CAREWare (i.e., VLS, Gaps in Care, HBV Immunity, etc.) and follow up as needed ( i.e., monthly meeting with Louisiana Department of Health Linkage Staff)Monitor and update randomized Eligibility Report pulled from CAREWare from BRTGA.Complete monthly billing and submissions based on established guidelines.Run monthly reports (i.e., BH, Phases, and outpatient ambulatory care) from the EHR (i.e., Athena), enter data into CAREWare; submit based on established protocols.Resolve any data discrepancies in CAREWareTrain staff on CAREWare use.Assists with trending data for submission to grants and federal reporting.Analyze client-level utilization data (RSR Reports) for accuracy and consistency with provider reports.Remains knowledgeable of National and State trends in Ryan White services and programs.Assists with planning community events that support persons living with Hepatitis C or HIV/AIDS and their families.Participates in Quality Management, Performance, and Improvement activities.Appropriately manage phone calls via the office phone.Appropriately manage designated RW fax system based on established protocols.Serves to assist the program director in office administration tasks such as supply ordering, meeting organization, and some event/activity planning.Maintain RW Program Policy, Protocols and Forms binder.Assist RW Program Manager or designee with managing RW mailbox.Performs other duties as assigned.MINIMUM REQUIREMENTS:

A bachelor's degree in healthcare/healthcare administration, public health, information technology, information systems, or a related filed.Be willing to travel within the established service area and to attend required trainings/conferences/and seminars.Must have in-depth knowledge of data management systems, principles, and practices including but not limited to data governance, integrity, security, and quality.Must have excellent communication (i.e., verbal, written), and time management skills.Direct or indirect experience with the Ryan White program is preferred.Must be proficient in relevant software and tools for data management, analysis, and reporting including database management systems, spreadsheet software, and analytical and statistical tools.Familiar with an EHR system is preferred.Experience in collaboration with interdisciplinary healthcare teams.Experience in ambulatory care setting preferred.Must have the ability to work both as a team member and work independently and seek guidance when necessary.Must have good interpersonal skills with ability to relate to diverse groups of people and people on all levels.Exceptional organizational, data analysis and research skills.Ability to maintain meticulous records.Ability to manage multiple concurrent deadlines.Must be proficient in relevant software and tools for data management, analysis, and reporting including database management systems, spreadsheet software, and analytical and statistical tools.SUPERVISES:
