SUMMARY:

Nurses provide leadership support to the Care Team by collaborating with team members and performing high quality, cost effective medical care essential to the assessment, promotion, maintenance and restoration of Ryan White patients’ health and wellness consistent with Southwest Community Health Center (SWCHC) policies and missions.

SPECIFIC DUTIES AND RESPONSIBILITIES:

All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA)

Manage immunization inventory for clinics and assist staff with re-ordering immunization supplies in timely fashion

Develop Anal PAP smear program for all Southwest sites

Assist providers with treatment of STI’s in office, (same day)

Assist with PrEP and perform three-site testing for Gonorrhea/chlamydia

Work directly with Principal Investigator on clinic trials

Assist with recapture efforts to re-engage persons lost to care

Work with OBGYN to promote PrEP for women campaign

Work with PEDS to promote rapid PrEP campaign

Train LPNs in internal medicine to be cognizant regarding HIV care

Assist in obtaining any outside records from specialists for the Clinicians before upcoming appointments.

Work closely with Clinicians for new referrals and ongoing patient care and coordinate specialty services for patients with internal and external providers.

Assist with initial patients by documenting Medical History, Medications, Allergies, and other relevant details in collaboration with the Clinician.

Conducts Nursing Triage on the phone and in the office, collaborating with the Nursing Team and Clinicians as needed.

Administers medications/vaccines and performs clinical procedures per standing order or clinician written order.

Perform medication reconciliation by phone or in office for established patients.

Performs screening tests and order routine lab work with medical standing orders

Completes specific treatment or nursing procedures as ordered by the provider

Assists in providing outreach services to patients who miss their appointments

Responsible for preparing the patient for the physical exam according to specific guidelines and prepare necessary equipment for the procedure

Conducts assessments and interventions identified in panel management meetings.

Ensures established quality assurance, CLIA waived tests, Joint Commission, Chronic Care model standards and are performed according to policy.

Provide Health Education by phone or at the end of an office visit.

As needed, conclude the office visit with a review of the after-visit summary and any relevant interventions.

Coordinate all phone/electronic message follow-up and place calls to patients to answer questions, conduct an assessment, or review lab results, as directed by the PCP.

Assist in completing patient forms, as needed.

Attends mandatory monthly Nursing supervisory meetings and occasional weekend trainings as required

Documents all patient calls in the EHR and creates nursing interventions to help patients meet goals outlined in the provider's care plan.

Provides education to all patients on the importance of medication compliance, diet, exercise etc. and ensures patients are following provider instructions for upcoming visits.

Other duties as required/designated

QUALIFICATIONS:

Holds a current Connecticut Registered Nurse’s license with 2 years of HIV experience and familiarity with Ryan White services

CPR certification (BLS)

Strong analytical skills and critical thinking

Excellent interpersonal communication skills and the ability to work with a diverse work force and client base

Proficient in MS Word, Excel, and PowerPoint

Experience with EHR documentation

Preferred: EPIC experience

EDUCATION:

Graduate of an accredited nursing program.

Generally, the working conditions are good with little or no exposure to extremes in health, safety hazards and/or hazardous materials. This person must have the ability to travel as required to work with staff.
