Summary:
The purpose of this position is to assist the Director of Case Management in the day-to-day operations of the Social Workers in the Case Management department. The Manager, with their inherent knowledge of the social worker position, will assist in the hiring, coaching, mentoring, and monitoring of performance of the social work team. The responsibilities of the Manager may include a patient care assignment, based upon the staffing needs of the department.
Responsibilities:
Assists with scheduling of staff and daily assignments to ensure adequate coverage of all units/floors with available budgeted staff.
Assists with the development of a comprehensive orientation process for all new hire Social Workers and is responsible for onboarding of new hires and evaluating competency.
Serves as a liaison to community post-acute providers and community referral resources.
Assists in evaluating Case Management program outcomes, assists with review and analysis of data to identify and address variances in outcomes/performance.
Conducts performance evaluations and identification of job goals for Social Work staff.
Institutes performance improvement strategies for those Social Work staff who are experiencing difficulties in performing job-related duties. Provides coaching as first level strategy to improve performance.
Participates in development of department Policies and Procedures.
Works collaboratively with Manager of RN Case Managers to ensure efficient and effective department operations.
Maintains contemporary professional knowledge and education.
Researches and promotes adaptation of best practices to ensure quality patient care and optimal outcomes.
Demonstrates regular, reliable, and predictable attendance.
Performs other duties as required.
Education:
MASTER'S LVL DGRE
Other Information:
Masters Degree in Social Work
LMSW with current CT license
3-5 years hospital based social work experience/or equivalent experience in community-based facilities (SNF/Community/MH/CPS/APS)
Experience in psychosocial and therapeutic counseling
Strong interpersonal and communication skills
Ability to form positive, collaborative relationships with interdisciplinary staff, patients, and families
Ability to negotiate effectively with internal and external providers
Ability to manage multiple competing priorities
Strong analytical and problem-solving skills
Preferred: CCM/ACM-SW certification
Working Conditions:
Manual: significant manual skills/motor coordination & finger dexterity
Occupational: Little or no potential for occupational risk
Physical Effort: Sedentary/light effort. May exert up to 10 lbs. force
Physical Environment: Generally pleasant working conditions
