JOB SUMMARY AND QUALIFICATIONS

The Case Manager’s primary duty is to provide case management to the families who are residing in the Family Emergency Shelter. The Case Manager will assist in ensuring effective daily operations of the shelter as well.

WHAT YOU SHOULD HAVE FOR THIS ROLE

This position requires a Bachelor’s degree in social work or related field or at least five (5) years of experience in the field and no degree would qualify for this position. Requires a demonstrated ability to provide a reasonable combination of the following knowledge, abilities and skills: family needs assessment, knowledge and ability to access local resources, advocacy, counseling, and working cooperatively with local service providers. Must have a commitment to the principles of the project. Excellent verbal and written communication skills, high degree of organization, and creative problem-solving skills. Must have personal automobile, valid driver’s license, liability insurance, and be willing and able to travel within the full service area. Must complete required case management and (Critical Time Intervention) CTI training within 90 days of hire.

RESPONSIBILITIES

Provides outreach to community partners and represents VOA values in interactions with community partners to increase awareness of and recruit landlords to work with our client families in the program
Provides case management duties, including:
Perform assessment and developing/monitoring case plans and conducting necessary follow-up activities.
Establish linkages with appropriate agencies and service providers in the area/community.
Provide referrals and coordinating with community partners and other service providers to assist participants in accessing health care services and daily living services.
Decide how resources are allocated to participants on the basis of need and eligibility.
Educate participants on issues, such as supportive services availability and participant rights.
Provide assistance to participants in obtaining public benefits.
Provide assistance to participants in coordinating or troubleshooting issues preventing them from accessing public benefits.
Provide/refer for supportive services to participants, including personal financial planning services, transportation services, income support services, fiduciary and representative payee services, legal services, child care, and housing counseling.
Complete required documentation (including progress notes) within 48 hours of contact
Complete and submit the required documentation for entry into the Homeless Management Information System (HMIS) within 48 hours of entry, transition, or exit. 
Demonstrate good clinical judgment in decision making regarding participants.
Demonstrate ability to relate to participants and their families in a culturally appropriate manner.
Performance Quality Improvement (PQI) duties as assigned by supervision & PQI Committee.
Facilitate the Critical Time Intervention (CTI) evidence-based model to program participants.
Work in partnership with other Case Managers, Housing Support Technicians, and Mobile Outreach Workers when needed; and participate in regular case consultation.
Advocate for the participant, integrating cultural values into their case plan.
Identify systemic barriers and communicates with organizational leadership about these barriers to work collaboratively to find viable solutions.
Comply with all policies and procedures of the program and the Council on Accreditation.
