A large managed care organization has a position for a Board-Certified Nurse Practitioner, must have Medicaid experience along with current resume providing last seven years of employment.Must have related work experience, including data entry along with excellent communication skills, and professional demeanor. Interact with internal and external customers in a courteous manner in person and via telephone.PAY:

2-3 days/week, 10-15 visits/week minimum.LOCATION:

Atlanta, GA***We need help with wellness exams that include doing A1C and using portable eye camera; this will be a main focus for them to conduct the in-home testing.***Travel to patient homes within Atlanta, GA area.Ideally the NPs would already be enrolled in GA Medicaid so to know that the suppliers may want to ask the NPs if they already see Medicaid patients for other jobs where they work or have worked previously.Education Required:Training as a Family NP is required. Can consider adults if no pediatric patients will be seen.Candidates must be board certified by the ANCC or AANP.Must have active/unrestricted license in state of position opening.New graduates will be considered.Must be able to be credentialed by Medicare, Medicaid, and other Private Insurance Companies.For states/markets that require state specific prescriptive authority in addition to the Federal DEA registration, NPs must be able to see patients without the physical presence of a collaborative physician (where such collaboration is required).HRS Qualifying Guidelines:Perform health risk assessments which are essentially history and physical done within the patient’s home or on occasion in nursing home or other health facility.Clinical documentation of acute, chronic, and long-term healthcare issues through patient encounter in EMR system; Documentation must be accurate, high quality, and completed in a timely manner.The provider will ensure all appropriate consent forms are signed and dated.Providers will have frequent interaction with patients, coders, schedulers, and supervising physicians.The provider will educate patients and/or patient’s family on chronic medical conditions, preventative care, and medication adherence compliance.The provider will complete referrals to case management, nursing, pharmacy, etc. as needed.Providers will participate in clinical meetings and compliance training which are primarily web based; however, provider will occasionally be required to attend on-site training.The provider will not be required to prescribe medicine nor order any lab or diagnostic testing.The provider will occasionally need to obtain samples for diagnostic screening tests such as finger stick for HgbA1C, LDL, and Lead levels; urine analysis may also be required based on client need.Provider will be responsible to review patient records for past and present health issues.Providers will need to collaborate with the supervising physician and other members of the health care team.The provider must be able to interpret laboratory data and diagnostic testing when available to assist in diagnosis of medical conditions.As part of the assessment, the provider must do ADL and Fall Risk evaluation, Depression and Nutritional Health Screening, BMI evaluation, and medication reconciliation.Performs other duties as assigned by supervising physician such as perform examinations using equipment for diabetic retinal exams, bone density, etc.Assessments will be added to weekly timecards when documentation is completed (using SSRS report).Scheduling based on needs and NP’s availability – this is not a full-time position and there are no guarantees of weekly needs. Needs fluctuate throughout the year.
