Family-NP - Community Health Center is seeking an experienced Family Nurse Practitioner for a Part Time (20 - 28 hours) Benefited Outpatient Position on Cape Cod.
The Nurse Practitioner provides comprehensive outpatient care, sharing responsibilities and collaborating with the physician in the care of patients.
The Nurse Practitioner will be responsible for providing services that include:Assessing, diagnosing, prescribing, treating, and patient/family education.Diagnosing and treating acute and chronic illnesses.Performing age-appropriate history and physical exams.Ordering and interpreting diagnostic and therapeutic tests relative to patient’s age-specific needs.Prescribing appropriate pharmacologic and non-pharmacologic treatments to patient’s age-specific needs.Monitoring the effectiveness of treatments.Providing education on acute and chronic medical conditions.Managing chronic medical conditions.Facilitating appointments to specialists.Facilitating appropriate chronic care management (i.e. Diabetic eye exams, foot exams, and nutritional counseling) and preventive medicine.
