Description:

To ensure quality, compliance, and safe delivery of home care nursing services as indicated by Agency policies, State and Federal laws/regulations.

The Director of Nurses oversees and coordinates all aspects of the client’s home care and interacts with other disciplines as needed. The Director of Nurses oversees and provides professional care to the client, as indicated by Agency policies, State and Federal laws/regulations. In-home admission visits and eligibility determinations are part of the DON duties, as is the development of the Care Plan. The DON also prepares paperwork necessary for Agency policies and State/Federal laws.

Reporting Relationship: Reports to Grey Handy and Executive Director.

Qualification/Prerequisites:

Is a graduate of an accredited school of nursing and is currently licensed as an RN through the State Board of Nursing.

Has a minimum of one year’s acute nursing experience, prefer minimum of one year supervisory experience.

Has a vehicle that is insured in accordance with state and/or Agency requirements and is in good working order.

Has excellent writing and charting skills.

Must pass a background check, fingerprint clearance, and drug screen.

Responsibilities/Activities:

Develops and maintains a good working relationship with all business professionals.

Completes an annual skills checklist and evaluation for all nursing and HHA staff.

Initiates Nursing or Home Health Aide Care Plan and associated paperwork, in accordance with Agency policies.

Evaluates and revises the care plan as necessary and as directed by the specific client program, participates in development and implementation of client care plans by attending ISP and IDT meetings as appropriate, or assures someone from the Agency attends the meetings.

Makes referrals to other agencies or services as needed.

Ensures the nurses, HHAs, and PCS employees receive a supervisory visit in the home a minimum of every 60 days.

Ensures required training is completed for nurses and HHAs upon hire, quarterly, and annually.

Participates in in-service training programs and staff meetings.

Serves as a liaison between clients and other health care professionals.

Provides reports to the client’s physician and case manager when the client’s condition changes or there are deviations from the Care Plan.

Addresses and reports patient concerns and complaints according to the Agency policies and procedures.

Reports any abuse, neglect, or exploitation within 24 hours, as per policy.

Submits documentation as per Agency policy.

Participates in clinical record reviews to ensure Dr.’s orders, medication profile updates, authorizations, and care plans (CM 485s) are current.

Complies with Homecare Agency Compliance Plan, Infection Control Policies, and HIPAA Policies.

Complies with the Agency’s dress code.

Complies with all State, Federal, and local regulations as set forth by the NM Department of Health Developmental Disabilities Medically Fragile Waiver Program and Medicaid programs as directed by the Agency Licensure policies.

Projects a professional attitude towards self, clients, and other staff members.

Assumes other duties, as requested by the Agency Manager, Administrator, or Governing Board.

Medical issues and nursing procedures.

Professional standards principles.

Other health care disciplines and their role in client and family care.

Home safety for situations, including physical and psychological dangers, for self, clients, and other Agency staff.

Case Management practices.

Clerical procedures such as maintaining records and completing forms.

English language.

Is self-directed with the ability to work with little supervision; has good organizational skills.

Pay: $35.00 - $40.00 per hour

Expected hours: 20 – 40 per week
