Remote but will conduct home visits in LTSS population members' homes. Need to reside in Tidewater, VA (Norfolk, Virginia Beach, Suffolk, Portsmouth and Chesapeake, VA).As an

with Sentara, you will work collaboratively with patients, families, and the medical team to provide the resources needed to prevent hospital readmissions. Your primary duty will be customizing care for patients, which includes assistance with home health, long-term care placement, home medical equipment, and community resources. REQUIRED
Registered Nurse/RN License in Virginia/Multi-state/Compact
REQUIRED
Specialty certification
REQUIRED
within one year of eligibility (ACM, CCM, CCCTM or RN-BC) for Integrated Care Management departments. Certification based on specialty area required within one year of eligibility for other service lines.
Preferred qualifications:3 years Case Management experience preferred.
RN-Associate's Degree
RN-Bachelor's Level Degree
RN-Diploma (Non-degree)
RN-Doctorate Level Degree
RN-Master's Level Degree
