This position is eligible for EDRP, a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of your start date. Approval, award amount (up to $200,000) and eligibility period (1 to 5 years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. The HUD-VASH Case Manager is responsible for independent coordination of case management services for Veterans receiving housing provided through standard and tribal housing vouchers. This role includes:Making independent decisions and designing policy and procedures for care delivery in a resource-scarce environment.Working closely with community resources to help Veterans obtain care and services.Providing intensive case management services focusing on housing, mental health problems, psychosocial needs, and barriers to treatment.Conducting psychosocial assessments to identify "at-risk" factors and developing treatment plans.Serving as the facility subject matter expert on family-based interventions and advocacy for homeless tribal Veterans.Mentoring and training personnel in service delivery to homeless tribal Veterans.Reviewing agreements between Tribes, landlords, Tribal Housing Authorities, and the VA for HUD-VASH placements.Conducting on-site visits of HUD-VASH dwellings to ensure compliance with local housing codes.Coordinating care and serving as an advocate for enrolled Tribal Veterans and their family members.Work Schedule:

Monday through Friday, 8:00 am - 4:30 pm.Requirements:

Experience with the homeless population, VA experience, and case management experience.Physical Requirements:
