A Brief Overview:Provides direct social work assessment and services to patients and families. Shall act in accordance with hospital and department philosophy, the NASW code of ethics, federal, state, and local laws, and other regulatory bodies such as HFAP.
What you will do:Demonstrates a commitment to the mission of Swedish Covenant Hospital and demonstrates a service orientation and adheres to all responsibilities and standards of the Hospital.Performs psychosocial assessment/evaluation of patients and significant othersImplements a plan of care to attempt to resolve psychosocial problems.Completes and maintains department records and statistics.
