Palliative Care Referral Nurse

Job Description:Job Summary:The Palliative Care Referral Nurse will work with the Palliative Care team to support clinical staff and physicians in the management of the continuum of care for palliative care patients. This includes assisting with patient evaluation, level of care determination, patient management, and goals of care conversations. The RN will assist with the education of staff and physicians.Principal Duties and Responsibilities:Collaborates with physicians and clinical staff to define and implement the plan of care for patients and families. Performs referral assessment of the patient in order to determine patient and family needs and goals of care. Demonstrates expert ability in clinical decision making regarding the palliative care patient population. Fosters communication and collaboration within the healthcare team, as well as with the patient and family. Facilitates transition to the appropriate level of care. Collaborates with the Hospice team to meet patient needs.Serves as a clinical leader and resource to staff and administration for patient care needs, resource recommendations, equipment and tools to enhance care, policy and procedure interpretations, and creative problem solving.Assists with the education and development of staff through informal and formal teaching opportunities. Provides unit-based in-services for those units responsible for the palliative population. Participates in unit huddles and rounding to review patient goals and progression with their plan of care. Provides education to patients and families about their condition, illness, and anticipated course of the illness process.Participates in clinical research at least through the utilization of research findings in planning, implementing, and evaluating preventive, rehabilitative, and supportive measures.Participates in required Palliative Care team meetings and Palliative Care Certification as needed.Minimum Education, Training and Experience Required:Experience: 3-5 years acute care experience preferred.Experience: one year Hospice/Palliative Care preferred.Education: BSN required along with active Kentucky license.Certification: CHPN upon hire or required within 2 years.BCLS certified.If you would like to be part of a growing family focused on supporting clinical excellence, teamwork, and innovation, we urge you to apply now!Baptist Health is an
