Job Summary:

To lead the coordination of housing and assisted living services for the facility.

Job Qualifications:

Preference given to experience in managing staff, administration, or the practice of assisted living services in the residential setting.

Work effectively with minimal supervision, strong communication skills, and ability to lead a group of people in completing tasks.

Must meet and maintain requirements of a certified Home Health Aide.

Meet the physical job demands of the position.

Hours of Work:

Full-Time. Works weekends and holidays, as necessary. Will make periodic visits to all shifts. On call status 24/7 as needed.

Duties:

PUBLIC RELATIONS:

Accepts inquiries and provides tours to potential residents and their families.

Maintains good relations with physicians, clinics, social workers, etc.

Has business cards/brochures readily available for visits/tours.

Has an active marketing plan with a calendar of scheduled events to advertise and promote the Company.

Provides effective communication with other individuals (i.e. courtesy when answering the phones, assists visitors, guests, and residents in finding their way around the campus).

Serve on appropriate community committees as needed.

Weekly public relations utilize 12 hours per week or as needed.

RESIDENTS:

In charge of the coordination of resident care: personal, medical, emotional, and spiritual.

Ensures Resident Care Conferences are scheduled twice per year for each resident or more frequently if needed. Assists with conducting these care conferences.

Reports Vulnerable Adult issues to the County/State and Ombudsman as needed.

Offers resolution or a visit to any dislikes or concerns of residents or family members of residents.

Ensures any grievance/concern is documented and communicated on weekly management report and to facility RN.

Incorporates customer-oriented approach into daily routine (i.e. using manners, politeness, follow up on concerns and reporting to appropriate department).

Ensures highest level of customer service with daily walk-throughs, periodic audits/surveys, and timely follow-up with concerns.

Visits with residents routinely regarding cares, activities, menus, etc.

Facilitates admissions along with RN.

Responsible for admissions and discharges with RN assistance.

Ensures bath schedule is adhered to.

Monitors the system for the residents' personal items being labeled and stored properly.

Maintains resident's confidentiality and respect according to HIPAA and Resident Rights.

Responsible for ensuring a proper welcome is given to each resident and/or family upon admission.

Monitors the ongoing activity program for individual and group activities.

Follows the resident's specific plan of care to ensure highest level of health, safety, and quality.

Follows and adheres to Resident Rights.

Ensures call lights are answered in a timely manner with courtesy and respect, knocking before entering the room and introducing self to the resident and/or family/guests in the room.

Ensure dignity and privacy is maintained while providing service to the residents.

OPERATIONS:

Assists in development and implementation of policies and programs.

Assures compliance with all applicable state and federal regulations.

Assures facility policies and procedures are current and followed.

Monitors facility census and Case Mix(Acuity) and assures staffing levels are appropriate.

Assures supervisory visits are conducted in a timely fashion and documented.

Assures service plans are current, updated when necessary, and changes are shared with Housing Manager for proper billing.

Produces work schedules to meet the needs of the facility and in accordance with policies and procedures.

Develops a good working relationship with the facility RN, physicians, county social workers, discharge planners, vendors, and other agencies.

Responsible for ensuring Housing Manager and/or RN is notified of changes in resident status, including falls, medication errors, and Vulnerable Adult issues.

Completes staffing schedule and posts timely as outlined in policy/handbook.

Reviews facility contracts and renews as appropriate. Ensures proper documentation is in place.

Facilitates general orientation for all new hires, ensures department-specific orientation is completed as outlined.

Assures proper documentation/charges are communicated to billing office in a timely manner each month. Assures any changes or updates are properly communicated.

Follows proper Infection Control policy and procedure.

Follows safety procedures, policies, and manufacturers' instructions applicable to assigned duties.

Responsible for prevention of unsafe work situations.

Attends training and in-service opportunities.

Oversees the completion and implementation of the facility calendar including activities, appointments, and daily/monthly food dietary menu and ordering.

Monitors the development, implementation, and organization of the food service program.

Ensures daily/monthly dietary menu and activity calendars are posted.

Ensures monthly updates are sent to County Case Managers.

Verifies resident billing is accurate; assists responsible parties with any questions.

BUDGET:

Responsible for development of yearly budget for facility.

Offers input for capital equipment budget.

Adjusts staffing levels as census and resident needs fluctuate.

Has a basic understanding of the information included in the budget, reviews it on a monthly basis to ensure expenditures are in line with budgeted amounts and responds appropriately.

Monitors hours worked, overtime, expenses, spending, and has a general awareness and consideration of the finances year to date.

Monitors use of supplies and attempts to minimize waste as much as possible.

Participates in the control of cost-effective services that include proper use of equipment, appropriate use of supplies, and effective/efficient organization of resident care duties.

Ensures residents have an approved payer source; monitors late payments/missed payments and addresses those concerns.

BUILDING, GROUNDS, and EQUIPMENT:

Responsible for the building; cleanliness, repairs, replacement of fixtures and equipment.

Plans with supervisor for capital items annually.

Responsible for notifying corporate Maintenance of any repairs, replacements of fixtures and equipment over $300.00.

Submits request to Maintenance Department in a timely fashion to request repair or maintenance.

Ensures grounds, lawn, snow removal, upkeep, flowers, and shrubs are maintained and taken care of; directs appropriate staff to complete tasks as necessary.

Develops & participates in disaster plans for the facility and conducts and documents monthly fire drills & yearly severe weather drills according to policies and procedures. Coordinates with maintenance staff.

Ensures a preventative maintenance program is in place and is being followed.

Responsible that storage area is kept neat and organized to promote a safe environment.

Observes for environmental cleanliness; picks up and discards any loose garbage, cleans and tidies up resident's living areas, wheelchairs, walkers, or other areas as necessary throughout the work area, including disinfecting when necessary/scheduled.

EMPLOYEES:

Oversees the interview, hiring, and onboarding of new employees.

In charge of training, coaching, counseling, and discipline employees related to general customer service and general work performance (i.e. answering phones, attendance, punctuality, respectfulness, following policies and procedures).

Develops and ensures good working relationships with staff and co-workers.

Monitors and ensures compliance with current job descriptions and staff performance review/evaluations for all staff in the facility according to facility policy and state regulations (Review monthly with the manager that evaluations or job descriptions are complete).

Always wears name tag.

Monitors staff turnover and retention.

Monitors and evaluates unemployment cases.

Annual benefit and wage review.

Responsible for provision of ongoing in-service training for employees.

Assures annual in-service training for employees follows company and state regulations.

Assures compliance with professional licensure for applicable staff.

Monitors day-to-day performance of staff as it relates to general work duties and responds appropriately to both positive and negative outcomes.

Ensures proper documentation is processed and provided to the payroll department for accurate pay procedures.

Monitors and assures compliance with current job descriptions and staff performance review/evaluations for all staff in the facility according to facility policy and state regulations.

Maintains Assisted Living Director license.
