Graduate of an accredited school of professional nursing program with a current license as a Registered Nurse in the state of Minnesota.Minimum of two (2) years of nursing experience in post-acute (long-term care, assisted living, memory care or home care)Previous supervisory/management experience in a health care settingPreferred Qualifications:

CPR certifiedBachelor's degree in nursing preferredAbout the Job:

The Director of Healthcare Services/Director of Nursing (AL) is a member of the key leadership team at The Homestead at Rochester. The position is primarily responsible for all clinical care and services provided to assisted living residents and home care clients. High quality care, customer service and team member satisfaction are three critical responsibilities of this role.Responsibilities:

Administration

Participates in campus wide strategic planning activitiesPartners with the Licensed Assisted Living Director to assure AL residents receive high quality care and servicesPartners with the Executive Director for the development and growth of the campus based home care agencyAssures compliance with all state, federal and local laws pertaining to assisted living and home careProvides leadership to all clinical assisted living and home care staffResponsible to assure staff education and monitors for complianceReviews policy and procedures annuallyRecommends new policy and procedures or changes to current policies to improve quality of care, meet state regulations and assure efficiency and effectiveness of servicesAssists in planning and development of assisted living and home care annual budgetsResponsible for risk management and completion of incident reportsReviews all incidents involving assisted living or home care residents and is responsible for investigation and timely notification of the Licensed Assisted Living Director and state officials according to the Minnesota's Vulnerable Adult Protection ActProvides support to the facility by being available for, or assuring nursing availability, for on call servicesProvide staff direction through general meetings, timely personnel evaluations and productivity expectationsEnsures accuracy in documentation and appropriate billing for all care and servicesCoordination of Services

Introduces home care services to new clients and familiesConducts pre-admission screening of prospective residents and home care clientsPerforms initial and ongoing assessment of physical & psychosocial needs and coordinates with other departments, agencies and care providers as neededMaintains ongoing communication with families, physicians and other care providers regarding resident or client status including condition changes, reaction to drugs, treatments or other incidentsPartners with residents and clients to develop a plan of person centered care/servicesEnsures/coordinates necessary ancillary services and appointment scheduling to ensure that needs are metEnsures appropriate documentation in the resident or client recordInitiates a resident or client service plan according to their individualized needs and as prescribed by Physician, medical provider and/or VOANS policyProvides resident/client and significant others education on conditions and treatmentsQuality Improvement

Establishes and leads quality improvement planning teamsIdentifies data collection requirements and reports to evaluate the effectiveness of the programs and to ensure quality of client/resident careIdentifies systems to recognize resident/client needs, respond to resident/client needs, and to measure the outcomes of nursing interventions. Utilizes this data to provide direction for improvementsProvides training, supervision and monitoring of staff in the administration of medications, to include frequently auditing medication sheetsSupervises the maintenance of resident charts, review documentation performed by care giving staff.Human Resources

Makes health care staff hiring decisions for assisted living and home careParticipates in orientation and ongoing training of team membersEnsures staff development, in-service education and online training programs are completed to ensure competency of staff and to meet regulatory requirementsCustomer Services

Possesses a strong customer service orientation in nursing practice, care and servicesImplements programs to ensure excellent customer service in assisted living and home careModels and holds staff accountable for the achievement of high quality customer servicesParticipates bi-annually in facilitating resident, client and family satisfaction survey and implements desired or necessary improvementsFollow ups with all resident, client and family issues or concerns and keeps Assisted Living Director and/or Executive informedAbout Rochester, Minnesota:
