Reports To:

Administrator or Executive Director

Supervises:

RN’s, LPN’s, CNA’s, PCA’s

Basic Qualifications

Education/Training:

A high school diploma/GED; licensed RN in state of Alabama

Experience:

Three years’ experience in a healthcare facility setting. Preferred assisted living/scalf setting.

Position Summary:

Must be able to verbally communicate effectively and accurately record events in writing. Must have an understanding of facility functions and the impact on residents. Must have the ability to perform a variety of simple repetitive tasks, some of which may involve unpleasant conditions. An individual must be able to assist residents with ambulation, bathing, dressing, eating, and grooming, and other activities of daily living as needed. Must possess the ability to exhibit patience and tact when dealing with residents, their families and visitors at all times and have a genuine interest in geriatric care.

Primary Duties and Responsibilities:

Complete monthly Nursing department schedule. Assist with call offs.

Complete incident reports (ensure incident reports and investigations are completed and that families and physicians are notified in a timely manner).

Obtain resident monthly weights (assist with monthly weight collection, enter weights into weight spreadsheets as needed, print weight sheets for monthly assessment binder, schedule dietary consult for residents with significant weight changes or special diets).

Obtain resident’s BPs, non-batch inventory, safety checks and room sweeps.

Oversight of batch meds once a month.

Assist Unit Coordinator monthly medication batch check in and daily check in of medications.

Review med book weekly after Unit Coordinator or ordering meds.

Notify family of VA meds that need to be reordered.

Create lab requisitions for lab orders and fax results to resident physicians.

Maintain tickler file with annual medical examinations.

Manager on duty rotation.

Assist Executive Director with new admissions (Complete admission exam, enter new resident information into Care Suite, create resident file, obtain new admission weight, etc.).

Assist with daily walk-in marketers, home health agencies, families and residents and schedule appointments for them to meet as needed.

Conduct resident chart reviews.

Processing physician orders (fax to pharmacy, conduct pending review in Care Suite, ensure meds are delivered).

Escort and/or assists residents to and from resident’s room, dining rooms and resident activities.

Assists in dressing and undressing residents.

Assists in bathing residents in tub or shower.

Combs hair, cleans and cuts resident’s fingernails and toenails, gives mouth care, assists in cleaning teeth and dentures and gives shampoos as needed.

Assists walking ambulatory residents as needed.

Responsible for required reports (including resident charts and other computerized reports or records).

Other duties as assigned.

Clinical/Technical Duties and Responsibilities:

Supervise and monitor patients in regards to patient safety, following applicable patient care policy and procedures.

Report any noted changes in resident’s condition.

Properly and accurately document all care given and supplies used.

May take and record temperature, pulse, blood pressure and respiration rate.

Ensure compliance with resident medication assistance guidelines outlined in SCALF Policy and Procedures.

Resident Services and Satisfaction:

Fully understand all aspects of residents’ rights; maintain the comfort, privacy and dignity of each resident in the delivery of services to them. Interact in a manner that promotes the emotional well-being of the resident.

Respond to patients and family member’s concerns with empathy and communicate with them in a courteous and respectful manner.

Maintain a professional demeanor and appearance on conformity with dress code standards.

Answer and refer telephone calls or other inquiries to ensure accurate and timely communications are facilitated.

Professional Development:

Attend in-service training.

Physical Work Environment:

Must be able to lift or carry up to 50 pounds occasionally and to exert 20 pounds of force frequently. Physical activity involves sitting, walking, standing, carrying, bending, lifting, crouching, reaching, pushing, pulling, grasping, feeling, talking, hearing and repetitive motions.
