LHH seeks a Case Manager in Hollywood, CA
This organization focuses on serving families and children who are homeless or at imminent risk of homelessness. This program aims to provide a centralized access point for families needing crisis services, shelter placement, rapid rehousing, permanent housing placement, linkages with schools and employment, physical and mental health care, and other supportive services.
Conduct intake and individualized needs assessment for all clients.
Collaborate with clients to develop an Individualized Service Plan (ISP) that addresses housing goals, income maintenance/increase, and other personal goals such as medical, mental health, substance use, financial resources, vocational, and social support needs.
Provide referrals, linkages, information, and support to resources that help clients achieve their ISP goals.
Evaluate clients' strengths and challenges in addressing short-term and long-term goals.
Conduct a 90-day review with clients after the initial intake to assess progress and make necessary adjustments to the ISP.
Maintain accurate and up-to-date client records using the HMIS system (HMIS experience preferred).
QualificationsA minimum of 1 year of case management experience.
Experience with HMIS preferred.
Must have a valid driver’s license and pass a TB test.
Ability to work one weekend day (Saturday or Sunday).
Strong interpersonal and communication skills.
Ability to work independently and collaboratively in a team environment.
Excellent organizational skills and attention to detail.
Pay Details:
$23.00 to $27.00 per hour
