Loan Repayment NegotiableSalary Range $111,609 to $139,512 / YearThe purpose of the Advanced Provider is to assist the physicians of the practice with clinical activities, surgeries and other direct patient care including diagnostic, therapeutic, and preventative care. Physician Assistants and Nurse Practitioners work under the direct supervision of practice doctors.Locations:

Up to .4 FTEAll Shifts are 12 hour shifts 7:00 am – 7:00 pm and 7:00 pm – 7:00 amPart time and/or per diem 4-6 shifts per monthEssential Job Functions:Perform patient history and physical examinationsDiagnose, treat, and manage acute and chronic health problemsOrder, and interpret laboratory and radiology testsPrescribe medication, including controlled substances as delegated in collaborative practice agreementDocument assessment, management, and education clearly, concisely, and legibly within medical recordDemonstrate strong communication skills with patients and significant others to provide feedback regarding findings and treatment plansAssess the patient and significant other’s psychosocial, emotional, and environmental needsConsult other health care team members in collaboration with physician to meet physician needs, while ensuring continuity of careProvide phone triage to assess patient’s needs and conditions and respond appropriatelyCoordinate patient care activities with other members of the office, hospital, or communityProvide education to patients, families, and community groupsDemonstrate compliance with documentation and coding requirementsMaintain and update skills and knowledgeWork with all members of the team to achieve patient-centered care as well as quality care by using the best practice standards and case managementContribute to scheduled team meetings to discuss/clarify roles, responsibility, processes, and patient issuesProvide care management by communicating with community agencies as appropriateParticipate in quality improvement activities and committeesHospital rounds to support the needs of the practiceOn-call duties including emergency call and telephone call, to support the needs of the practiceKnowledge, Skills and Abilities:Must be able to exercise a high level of independent judgmentExceptional service attitude and communication styleMaintains highest level of confidentialityExcellent written and verbal skillsDemonstrates clinical competency as defined by organizational standards and protocolBLS CertificationMaintain CME requirementsProfessional appearance and demeanorEducation and Experience:Current NYS license and board certificationCompletion of an accredited Nurse Practitioner or Physician Assistant ProgramCurrent BLS CertificationPhysical Requirements:Standing: 80%Reaching: 50%Lifting: Must be able to lift 40 poundsPleasant conditions: comfortable indoor climate with suitable equipment and tools to carry out the responsibilities of the jobPersons in this job may be exposed to communicable disease through patient care and interactionSystem Information
