Clinic RN

accurately and appropriately provides patient-centered care by means of health protection, promotion, optimization, and prevention of illness or injury. This role performs both independent and delegated nursing functions including: assessment, nursing diagnosis, planning, implementation, and evaluation.Clinic RN Overview

Key responsibilities include but are not limited to:Establishes measurable and achievable short- and long-term goals for patients.Implements nursing protocols based on the treatment plan, ensuring continuity of care for patient. Provides education to patient and caregiver(s).Evaluation of the patient's status and efficiency of the nursing care and modifies care plan as needed.Helps provide healthcare access through nurse visits, management of in-basket task (phone messages, MyChart communication, refill requests, results).Provides safe and timely telephonic triage for symptomatic callers, using system approved triage policies and protocols.Minimum Qualifications:

Current Minnesota RN Licensure and knowledge of specialty area.Associate's (ADN) or Bachelor's Degree in Nursing (BSN).Healthcare Provider Basic Life Support (BLS).Excellent assessment and care coordination skills.Clinic Location:
