Responsibilities:

Functions as part of the leadership team in the development of the organization’s goals. Develops, recommends, and administers agency policies and procedures.

Measures our clinical operations against Joint Commission requirements and drives clinical excellence throughout our organization. Maintains our organization in a “survey ready” state of operation.

Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel.

Stays informed about changes in the field of nursing and home health care. Attends Brightstar monthly online DON meeting. Shares information with appropriate organization personnel.

Attends Living Room Visits, making the initial evaluation. Regularly reevaluates the client’s nursing and care needs.

Prepares clinical and progress notes and summaries of care.

Informs the physician and other appropriate personnel of changes in the patient's condition and needs.

Counsels the client and family in meeting nursing and related needs.

Supervises, educates and coaches other nursing personnel/care-giving staff. Delegates assignments as necessary.

Assesses competency of clinical staff and ensures staff is assigned in a manner that promotes quality, continuity and safety of a client's care.

Provides care as defined within the scope of the state’s nurse practice act. Covers client shift as necessary.

Handles some of the after-hours phone support as necessary.

Plans and implements in-service and training programs.

Oversees the maintenance of client clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities. Maintains those records in compliance with local, state and federal laws.

Develops relationships with home health referral agencies.

Works with community emergency response organizations and local public health departments, as necessary.

Other duties as assigned.

Requirements

Current unrestricted Wisconsin RN license.

Minimum three (3) years of clinical experience with one (1) year experience in a home health agency, primary care clinic or health facility,

preferred .

Ability to start new infusion cases on occasion.

Reliable car/auto insurance.

Clean background and criminal record.

Understand regulatory compliance with local, state and federal regulations.

Adhere to HIPAA and maintain client confidentiality.

Demonstrated ability to supervise and direct staff.

Possess exceptional nursing assessment skills.

Comply with accepted professional standards of practice.

Proven ability to build quality and implement improvements within the operation.

Excellent interpersonal skills.

Analytic/Critical thinker.

Excellent customer service skills and ability to deal tactfully with clients, staff and community.

Organizational Skills.

Computer Skills (electronic charting; Microsoft Teams, Outlook, Excel, Word).

Case management skills are a plus.

Policy development is a plus.
