Case Manager - Adult Foster Care
What You Will DoConductcomprehensive initial home assessments, ensuring all necessarycaregiver credentials are in place and required home modifications arecomplete.Gather essential information, perform psycho-socialevaluations, and facilitate referrals to appropriate healthcare andcommunity services.Coordinate patient placement in compliance with established procedures.Collaborate with caregivers and the care team to define and implement individualized care plans.Perform regular home visits to supervise personal care services and review care plan progress.Maintain detailed and accurate case records, documenting significant changes and observations.Provide ongoing training and support to caregivers.Collaborateeffectively with nurses, primary care physicians, and other medicalproviders to ensure timely and appropriate delivery of consumerservices.Actively participate in discharge and transition planning.Conduct annual reviews to ensure the continued suitability of the qualified setting.Maintain continuous survey readiness and ensure compliance with all regulatory and accreditation standards.Perform other duties as assigned.
What You Will Bring (Required)
Bachelor's degree in social work, human services, or a related field.At least one year of experience working with elders or adults with disabilities.Valid driver's license and automobile insurance, as travel within Massachusetts is required.Demonstrated experience in case management, care planning, and assessment in a healthcare setting.Proficiency in common software and web-based applications.
Additional Qualifications (Preferred):
Master's degree in social work or a related field.Experience working in Adult Foster Care or a similar setting.Certifications in gerontology or case management.Social worker license from the Massachusetts Board of Registration in Social Work
