Key Responsibilities:
Conduct home visits to assess and create personalized care plans.
Train and support caregivers, ensuring they can meet their loved ones' needs.
Monitor and report on clients’ health, coordinating with their primary care physicians.
Facilitate smooth transitions and ongoing care for clients.
Maintain compliance with all relevant regulations and standards.
Qualifications:
Active RN license in the state of employment.
2+ years of experience in elder or disability care.
Valid driver’s license and the ability to travel locally.
Strong case management and care planning skills.
Proficiency in MS Office and case management systems.
Additional state-specific qualifications may be required.
