Clinical Support Nurse, Navigation Center

Responsible for telephone assessment of patient's needs under the direction of Cottage Navigation Center Leadership with collaboration of the clinical departments. Application of a variety of procedures and policies relating to both the standards and procedures of the nursing profession and those established by Cottage Health. Provides triage for patients with symptoms, advice, medication refills, required documentation for authorizations, and follow-up education consistent with patient's age and cultural needs. Responds to patient inquiries, prioritizing responses and routing urgent situations to a Physician, Physician Assistant or Nurse Practitioner (hereafter Provider). Maintains accurate and appropriate documentation in the electronic health record. Follows patient privacy in accordance with HIPAA and Cottage Health policies and procedures. Performs to the level of a licensed Registered Nurse as dictated by the State of California Nurse Practice Act and Cottage Health.

LEVEL OF EDUCATIONMinimum: Associates in Nursing

Preferred: Bachelor's of Science in Nursing Degree

CERTIFICATIONS, LICENSES, REGISTRATIONSMinimum: Current California RN license. American Heart Association (AHA) Basic Life Support (BLS)

TECHNICAL REQUIREMENTSMinimum: Familiarity with MS Office, Microsoft Outlook, and Electronic Medical Records

Preferred: Experience with Epic and CRM

YEARS OF RELATED WORK EXPERIENCEMinimum: 3 years clinical experience

Preferred: Minimum of one year of professional nursing experience in a clinic setting. Experience using call center phone systems. Training and experience in specialty that will be supported.
