Title: Youth in College Case ManagerReports To: Workforce Development and Education Manager

La Casa Norte’s mission is to serve youth and families confronting homelessness. We provide access to stable housing and deliver comprehensive services that act as a catalyst to transform lives and communities The Youth In College Case Manager at La Casa Norte is responsible for providing comprehensive academic and case management support to youth who are experiencing homelessness or housing instability and are enrolled in college. The role involves creating individualized academic plans, connecting youth with necessary resources, and acting as a mentor and advocate to ensure their academic success and overall well-being. The ideal candidate will be enthusiastic to help students, knowledgeable of the college process, and serve as a positive role model for the students.This is a full-time position working Monday through Friday, with one Saturday per month and some evening hours required, including on-call responsibilities.

Key Responsibilities:Case Management:Conduct comprehensive assessments of youth’s academic needs, strengths, and challenges.Develop and implement individualized academic support plans in collaboration with each student.Provide ongoing case management, including monitoring academic progress, attendance, and overall well-being.Work closely with the youth to set academic goals and milestones, providing motivation and support to achieve them.Maintain detailed documentation for each youth including case notes, Scholarship Ledger, in all required formats (ClientTrack, HMIS, paper file, etc).Academic Support:

Assist youth in navigating college systems, including class registration, financial aid, and accessing campus resources.Provide tutoring or coordinate tutoring services to help youth overcome academic challenges.Connect students with educational resources such as study groups, workshops, and online learning tools.Advocate for the youth’s needs within the academic environment, including liaising with faculty and college administrators as needed.Resource Coordination:

Identify and connect youth with additional resources to address barriers to academic success, such as housing, healthcare, mental health services, and financial assistance.Work collaboratively with other La Casa Norte programs and external partners to provide holistic support to each student.Facilitate access to scholarships, grants, and other financial aid opportunities to ensure the youth’s ability to continue their education.Mentorship and Advocacy:

Act as a mentor and positive role model for the youth, offering guidance and encouragement throughout their academic journey.Advocate for the youth’s rights and needs within the educational system and in broader community contexts.Foster a supportive and trusting relationship with each student, encouraging self-advocacy and empowerment.Program Development:

Assist in the development and implementation of program strategies and activities aimed at improving academic outcomes for youth in college.Participate in program evaluation and continuous improvement efforts to enhance service delivery and effectiveness.Contribute to the design and facilitation of workshops and events that support academic achievement and personal growth.Collaboration and Teamwork:

Collaborate with other La Casa Norte staff to ensure comprehensive service delivery to youth.Participate in regular team meetings, training, and professional development opportunities.Engage with community partners and stakeholders to strengthen the support network available to youth.Qualifications:

Bachelor’s degree in education, social work, counseling, or a related field.Minimum of 2 years of experience in case management, academic advising, or a related field, preferably with at-risk or homeless youth.Strong understanding of the challenges faced by homeless youth in accessing and succeeding in higher education.Excellent communication, organizational, and interpersonal skills.Ability to work independently and as part of a multidisciplinary team.Commitment to La Casa Norte’s mission and values.Preferred Qualifications:
