Job SummaryThe Herbert Wertheim College of Medicine is currently seeking a Sr Licensed Practical Nurse to join our team of professionals.

Duties Include:

Coordinates patient referrals to medical/surgical specialty physicians and/or outpatient services. Coordinates Neighborhood Help (NHELP) interdisciplinary team referrals. Coordinates and tracks clinical referrals for Care Transition Clinic (CTC). Leads the coordination of Jackson Health Center financial assistance referrals by working with providers, the outreach team, Jackson Health system, and students to aid patients in obtaining approval to close the loop on medical referrals.

Provides patient/family education and instructions as delegated by the Provider and/or outpatient referral source. Documents all instruction/education, and conversations with the patient into the medical record.

Reviews lab and other diagnostic study results and communicates critical or abnormal values to the Physician.

Provides patient/family education and home care instruction as delegated by the Physician and documents all instruction/education provided in the medical record.

Maintains patient and business confidentiality at all times and treats patients, the public and co-workers in a professional manner. Conducts daily medical record review activities to ensure that documentation is accurate, complete, timely, and appropriate.

Intermittently assists on the Mobile Health Center with vital signs, intake, phlebotomy, immunization administration, medicines, treatments ordered by the physician. Documents all care given in the electronic medical record.

Participates in Quality Improvement. Assists in the evaluation of the quality of patient care and patient satisfaction, makes recommendations for improvement, and formulates treatment plan to meet patient needs.

Performs other related duties as required or directed. The omission of specific duties does not preclude the supervisor from assigning duties that are logically related to the position.

Serves as back up to the Clinical Director of Operations with Americares/Direct Relief medication management. Responsible for appropriate dispensing and tracking of prescription medications.

Minimum QualificationsHigh school diploma or equivalent and three (3) years of related nursing experience; OR an equivalent combination of relevant education and/or experience.

Basic Cardiac Life SupportLicensed Practical Nurse

Desired QualificationsGraduate of accredited school of Practical Nursing.Experience preferably in a physician practice or clinic environment.Phlebotomy and Immunization Administration experience.Bilingual English/Spanish preferred.Experience working with Electronic Medical Records.
