JOB SUMMARY/PURPOSE
Directs the management of ambulatory nursing operations and the clinical practice of the ambulatory primary, specialty, and urgent care clinics in partnership with AVP- Operations and Senior Medical Directors. Oversees regulatory and safety activities. Develops improvement strategies by identifying and evaluating trends, needs, forecasts and established measurements to provide quality care. Designs programs and procedures, resolves issues, and implements change. Provides programmatic leadership and direction for the continuum-based operating model for care management and care coordination services for SMG.

EXPERIENCE/EDUCATION/QUALIFICATIONS
Current unencumbered Oregon RN license required.
Master of Science in Nursing degree required.
Five (5) years management experience in a healthcare or health plan setting required.
Three (3) years clinical experience preferred.
Experience in care coordination, care management, care transitions, population health, or disease management preferred.
