The Discharge Planner is a member of the inpatient behavioral health interdisciplinary team that assists the Social Workers with transitioning patients from the hospital setting to the next level of care. S/he will initiate contacts with a variety of community resources, implement linkages, and provide coordination and communication to facilitate discharge planning.The Discharge Planner will assist with completing referral forms, bed searches, and other paperwork as needed.Must be a self-starter with the ability to multitask, prioritize their workflow, work well under pressure, and work independently. Must be computer-literate for documentation purposes and skilled in the use of email and internet services. Will need to interact professionally with all members of the multidisciplinary team and the community.WORK EXPERIENCE REQUIREMENTS:1-3 years of healthcare experience and knowledge of programs, resources, and supports in Central Massachusetts are preferred.Experience in the mental health or substance use field is also preferred.EDUCATION REQUIREMENTS:Bachelor's degree in social work or human services field preferred.CPR (BLS) and CPI certification required within 30 days of hire.All qualified applicants will receive consideration for employment without regard to race, color, religion, sex, sexual orientation, gender identity, national origin, disability, or protected veteran status.We're striving to make respect a part of everything we do at UMass Memorial Health - for our patients, our community, and each other.
