Observe and report symptoms and condition changes of residents to physicians, director of nursing, and/or designee, resident’s representative, or other 3rd party providers, as necessary. And above all, provide excellent patient care.Primary responsibilities include ensuring qualified employees and contractors provide patient care and paraprofessional services, providing oversight of all patient care services and personnel, which include making patient and personnel assignments; coordinating patient care; coordinating referrals; assuring that patient needs are continually assessed; and assuring the development, implementation, and updates of the individualized plan of care.Manages and directs a staff ensuring safe, effective and appropriate home care services. Assists clinical staff in establishing priorities, setting goals and evaluating progress toward goals.Available at all times during regular business hours and as needed to provide support and assure quality care delivery to home care clients. Coordinate 24-hour R.N coverage. Provides on-call back-up for staff as needed.Attends case conferences and other clinical meetings to facilitate coordination of care.Provides support and direction to Agency staff, other health care professionals, clients and families related to appropriate and available health care resources.Assists with marketing, public relations and discharge planning by participating in Departmental meetings. Stays current on available community resources, health care costs and industry trends through self-education and access to outside educational opportunities.Ensures accuracy, completeness and timeliness of clinical documentation in accordance with Agency policies and procedures, regulatory requirements and industry standards.Identifies and implements changes in clinical and/or operational practice based on the findings of the Quality Improvement programs. Monitors open and closed charts regularly and participates in the quarterly clinical record reviews.Interprets and enforces human resources policies and procedures in a fair and consistent manner. Assists in the screening and interviewing of new Agency personnel and makes recommendations to Administrator.Attend all required in-services, trainings and meetings as required or requested. Adhere to attendance policy.Required Qualification:Graduate of an accredited school of professional nursing, BSN preferred.Current license as a Registered Nurse in the State of Florida.Minimum of two years nursing experience in an acute hospital setting or equivalent experience with one year of supervisory experience.Excellent verbal and written communication skills and strong interpersonal skills. Knowledge of the regulatory requirements at the State, Federal and local level and reimbursement requirements of governmental agencies and private insurance.CPR certified.For purposes of effective communication with residents, families and staff must be able to see, hear, speak, read and write English.Maintain ability to stretch, push, pull, kneel, reach, twist, lift unassisted at least 50 lbs and walk or stand up to eight hours.Salary: $80,000-$90,000A criminal history background check and sex offender registry check will be required for the finalist under consideration for this position.
