The Transition of Care Nurse (TOC) contacts the members upon discharge to ensure a safe discharge. The TOC Nurse will follow up with the member to ensure all discharge orders have been filled and PCP/specialist follow-up appointments have been scheduled.QUALIFICATIONS

REQUIRED QUALIFICATIONSRequired Work Experience2 years Nursing experience in a healthcare settingRequired EducationAssociate’s Degree in NursingRequired LicensesRegistered Nurse with a current, active, unrestricted nursing license in the state of Arizona.Required CertificationsMust have and maintain a valid driver’s license and clean driving record.PREFERRED QUALIFICATIONSPreferred Work ExperiencePrior HRA experiencePreferred EducationBachelor’s Degree in NursingESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES

Telephonic contact with the members within 3 days of dischargeMay require in-home visits to membersFacilitate completion of all discharge orders that have not been completedComplete TOC Assessments and surveysObtain data needed for the Medication ReconciliationFollow up with members up to 30 days post-discharge based on members' needsPerform all other duties as assignedThe position requires a full-time work schedule. Full-time is defined as working at least 40 hours per week, plus any additional hours as requested or as needed to meet business requirements.COMPETENCIES
