Salary: $130,000 - $130,000 per year

• Coordinate cost-efficient and reimbursable patient care under insurance plans.
• Review patient charts to ensure all services are provided per the plan of care.
• Participate in QAPI activities and quality improvement audits.
• Provide staff development, including orientation, supervisory field visits, and in-service training.
• Ensure accurate documentation reflecting patient care and compliance with physician orders.
• Participate in care coordination meetings to review patient progress and discharge planning.
• Supervise pediatric nurses, including joint visits and annual evaluations as per Department of Health guidelines.

Skills You Need

• Registered Professional Nurse license and current registration in New York State.
• BSN Degree from an accredited school of nursing; BLS required.
• 3+ years of experience in home care/CHHA environment; supervisory experience and experience with pediatric patients are a plus.
• Reliable transportation for travel within Brooklyn, Queens, and Manhattan.
