Our team is comprised of more than 1,500 primary care and behavioral health clinicians in more than 31 states. Our post-acute care clinicians receive best-in-class orientation and training, a dedicated EHR system, and access to ongoing clinical education.
For this opportunity, we ask that you be board certified in family medicine (FM) or internal medicine (IM), have a passion to serve active older adults, and a desire to lead.
Opportunity Overview:
Coverage area for full-time will be South Bend, Niles, and St. Joseph (3.5-5 days)
Coverage area for part-time will be South Bend, IN and St. Joseph, MI (1-3 days)
15-20 patients per day
Mileage reimbursement
Medical directorship stipend
Uncapped productivity bonus
Responsibilities:
Drive high-quality, safe, and cost-effective patient care in post-acute care communities
Collaborate with other clinician interdisciplinary teams and follow CMS guidelines
Work closely with hospice, palliative care, and home care agencies for safe patient transitions
Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines.
Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated salary range of $114,000 to $300,000 annually with no cap on productivity income potential and medical directorship.
