Monogram serves patients with CKD, ESRD, and polychronic conditions. MDs work from their home office via tele-health. Monogram MDs are quality driven physicians who are dedicated to improving the well-being, quality of life, and health outcomes for all our patients.Roles and Responsibilities

Utilize data tools to report, monitor, and improve quality and utilizationConduct retrospective reviews of hospitalizations and dialysis treatment episodes to ensure Monogram's care management services are effectively renderedOversee clinical strategies, evidence-based clinical care guidelines, and care management programs for effectiveness and efficiencyReduce unnecessary episodes of care, including hospital readmissionsPrepare clinical care reports in a timely fashionApply evidence-based criteria to determine appropriate care and lead case rounds with nurse practitioners and care managersAnalyze data to build member care plans and ensure care goals are achievedIdentify quality of care issues for health plan clientsOversee the execution of care management within the Atlanta marketAchieve clinical metrics and goals for delivering improved health outcomes at a lower total cost of care.Requirements
