Job Description

Under direction, incumbents perform a variety of specialized duties in support of programs for Juvenile 602WIC wards of the court who are removed from home and committed to Out of Home Placement (OOHP) and declared a 450WIC Non-minor dependents for Title IV-E funding for Extended Foster Care (EFC).

Essential functions include, but are not limited to:

Provide support in release planning for incarcerated justice involved youth at our Juvenile Justice Center that may include visiting the youth in custody and traveling.

Provide support in reentry planning for our Out of Home Placement Foster Care youth that may include visiting the youth and traveling.

Provide specialized case management services to eligible clients;

Conduct assessments to develop appropriate case plan; prepare summaries of clients' functional limitations, barriers, and/or needs; make referrals to community services.

Educate and help participants navigate through program expectations, policies and procedures, and service resources.

Partner or collaborate with other internal agency staff, community organizations, collaterals, and service representatives to provide clients with the services they need.

Participate in regular case consultations, including but not limited to Child Family Team meetings to discuss client assessments and case plan.

Compile and submit statistical information into case management systems and/or narrative reports.

Appear in court as needed and represents the department in matters involving 450 WIC compliance; submits supplemental information as required, prepares and submits court reports with recommendations for unit supervisor's approval.

Assists in the training of new social workers and may act as backup supervisor.

Serve as a liaison and represent the department at county or state level meetings; may prepare special reports and deliver lectures on extended foster care.

Travel in-person to visit clients at their extended foster care placements, schools, employers, churches, neighborhoods, recreational areas, and other places.

Attend trainings or conferences as needed.

Qualifications:

Education:

Possession of a baccalaureate degree from an accredited college or university.

Experience:

Two (2) years of social casework or employment services experience with a public or private agency/organization.

License and Certification:

Substitution:

Additional experience as described above may be substituted for the required degree on a year-for-year basis (up to a maximum of two (2) years). Thirty (30) semester or forty-five (45) quarter units equal one (1) year.

Possession of a Master's degree in Social Work or Counseling may be substituted for one (1) year of the required experience.

Desirable Qualifications:

In addition to meeting the minimum qualifications listed above, the ideal candidate will have demonstrated a successful track record of having the following knowledge and abilities.

Knowledge of:

Operations, services and activities of OOHP and EFC

Departmental programs, policies, procedures and terminology

Pertinent federal, state, and local laws, codes, and regulations as related to EFC and Title IV-E

Interviewing and investigative skills and techniques

Principles and practices of caseload management

Principles and procedures of record keeping and reporting

Ability to:

Prepare clear and concise reports;

Operate standard office machines;

Analyze and interpret information and adopt effective courses of action;

Gain cooperation through effective communication;
