The ideal candidate will be results-driven, team-and detail-oriented professional who is comfortable working on multiple tasks in a deadline-oriented environment.
Responsibilities:
Provide initial assessments and intake evaluations in a clients homeProvide individual, couples, family or group counseling within the scope of one's competencies.Provide crisis intervention.Develop and implement treatment plans and document services consistent with Center standards and/or insurance requirements.Collaborate with other providers, agencies, insurance companies and individuals in the client's network of care.Provide relapse prevention, discharge planning, follow-up and aftercare.Provide education, training, and modeling in basic daily living skills, social or parenting skills.Teach clients effective techniques for managing their medication as prescribed.Coordinate community resources, transportation for client, and referrals to appropriate resources including services within the Center.Assist clients in developing and strengthening their support network to accomplish goals and objectives.Produce, complete, and maintain billing forms, logs, data collection forms, clinical documentation and client records in accordance with Center standards and state and federal regulations.Ensure clients have appropriate medical coverage at all time.Accompany clients to community appointment, provides advocacy as needed.Schedule and maintain client hours sufficient to achieve individual / program billing targets.Requirements:
Master's Degree in the human service field of social work or counseling preferred.Minimum (2) year experience working with clients with disabilities preferred.Experience or training in related state and federal assistance programs also preferred.Licensed at an independent levelBasic computer literacy skills are necessaryValid driver's license and automobile insurance which meets Rhode Island minimum standards law, or the ability to obtain necessary transportation in order to perform the job responsibilities/tasks is necessary for community based positions.
