Carlsbad Lifehouse, Inc., a leading provider of mental health services in Carlsbad, New Mexico, seeks a qualified and compassionate Primary Care Provider (PCP) to join our esteemed team. In this vital role, you will provide comprehensive primary care services to a diverse patient population, including members of the general public and residents of our Lifehouse residential and sober living programs. Responsibilities:

Conduct comprehensive physical examinations and health assessments.Diagnose and treat a broad spectrum of acute and chronic medical conditions.Develop and implement individualized treatment plans encompassing preventive care, medication management, and referrals to specialists as needed.Educate patients on disease management, healthy lifestyle choices, and preventative measures.Collaborate effectively with nurses, other healthcare providers, and Lifehouse residential and sober living staff to ensure coordinated patient care.Maintain accurate and detailed electronic medical records (EMR).Demonstrate a commitment to staying current on the latest evidence-based practices in primary care medicine.

Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) license, Master of Science in Nursing (MSN) with a specialty in Family Practice (CNP), or Physician Assistant (PA) certification.Active and unrestricted license to practice in the state of New Mexico.Minimum of 2 years of experience in a primary care setting.Strong clinical skills in history taking, physical examination, and diagnosis.Exceptional communication, interpersonal, and collaboration skills.Patient-centered approach to care with a focus on building rapport and trust.Proficiency in EMR systems.
