JOB SUMMARY
Under general supervision of the Clinic Manager, provides screening and biopsychosocial, suicide risk, and other specific assessment tools in evaluation of clients, including individuals with more complex needs
Determines necessity for involuntary evaluation, completes 1013/2013 paperwork, and may partake in coordinating transfer to a receiving facilityProvides sound triage decisions, clinical formulation and diagnostic impressions based on clinical assessmentsDevelops client treatment plans in collaboration with the individualProvides individual, group counseling, family counseling, and other treatment modalities within an evidence-based frameworkCoordinates care with the treatment team and external resourcesDemonstrates clinical knowledge in mental health, substance use, and co-occurring disorders along with social determinants of healthPerforms duties from a recovery oriented, client centered, and trauma informed perspectiveDocuments services in the electronic health record according to agency standardsDemonstrates advanced clinical judgement in care provisionRegularly obtains clinical supervision and seeks additional supervision as necessary
DUTIES and RESPONSIBILITIES:
ASSESSMENTS
Performs thorough bio-psychosocial assessment of the less complex to more complex clients and documents the assessment within agency guidelinesUtilizes specific assessment tools in evaluation of the client as per agency guidelines.Utilizes information from multiple sources as appropriate in the formulation of the Interpretive Summary and in client driven treatment planning.Demonstrates strong clinical judgment in assessments, application of diagnostic criteria, clinical formulations, and sound triage decisionsDetermines necessity for involuntary evaluation, completes 1013/2013 paperwork, and may partake in coordinating transfer to a receiving facility.
ADVANCED THERAPEUTIC COUNSELING
Creates therapeutic relationship with clients and families/significant others.Conducts individual, family, group counseling/therapy, and educational sessions using evidence-based interventions.Demonstrates clinical knowledge in mental health, substance use disorders, and co-occurring disorder treatment along with social determinants of health.Demonstrates documented clinical proficiency (through education, training, or experience) in client engagement, interviewing, evidence-based practices, and recording of services.Documents clinical services concurrently and no later than 24 hours after service delivery (48 hours if providing a group) in an electronic health record.Demonstrates effective use of Telehealth as appropriate in care provision.
DEVELOPS and PARTICIPATES IN COMPLEX CARE MANAGEMENT / CARE PLANNING
Creates, communicates and coordinates treatment plans including those for clients with more complex and/or high-risk clinical presentations.Demonstrates expertise in care/planning through objectives, goals, and specificity of plan. Updates the treatment plan as the clients needs change.Serves as resources to staff in demonstration of up-to-date knowledge of Claratel Behavioral Health, community, regional, and state resources.Assist treatment team in problem solving regarding needed resources for complex and high-risk clients.Insures timely communication with providers through proper exchange of written and verbal communication per policy and procedure.Document referrals, discharges, and resources or other issues for problem resolution and/or utilization as required in agency policy.Effectively manages a caseload, assuring outreach and follow-up occurs as per agency policy and procedures.

Maintain licensure/certificationActively participates in clinical supervisionActively engages in behavioral health educ tional initiativesSeeks continuing education to meet current clinical needsMaintains clinical service delivery hour expectation.All clinical documentation must meet agency and regulatory guidelines.
EDUCATION AND/OR LICENSURE REQUIREMENTS
Current licensure as an LCSW, LPC or LMFT by the Georgia Composite Board of Professional Counselors, Social Workers, and Marriage and Family Therapists is requiredDemonstrates competency in using evidenced-based treatments for mental health and substance use disordersThe employee must be able to use an electronic health record proficientlyThe employees must demonstrate initiative, flexibility, and professionalism.
PREFERRED QUALIFICATIONS
Experience in a residential or outpatient setting with mental health or substance use programsExperience working with adults, children, and adolescentsExperience conducting group counseling/therapy services
WORKING CONDITIONS
Outpatient behavioral health clinic office settingMonday-Friday 8:15am-5:00pm with some evening and/or weekend hours (as determined by clinic needs)Ability to use a computer, telephone, fax, copy and scanning machine
