Key Position Details:Primary care team has 5 Internal Medicine Physicians, 1 Advanced Practice Providers, and 1 Psychologist36 patient contact hours per weekMinimal call coverage, phone only, RN triage supportOutpatient practice providing care to all ages, including growing familiesClinic hours are Monday- Friday 7:30am-5:00pm and closed Saturday-SundayEMR: Excellian is one of the most comprehensive electronic health record (EHR) systems in the nation.Minneapolis boasts being both cosmopolitan yet small enough that you can find a place to call home whether on a lake, golf course, or in an adjoining suburb. With the fast pace of activity there are professional sports, theater, or enough parks for a quiet walk. The area has numerous schools and colleges, both public or private, and enough teams for all weekend athletes to join.Job Description:Principle ResponsibilitiesWorks collaboratively with patients to improve their health and well-being.Practices in a thoughtful manner including consideration for cultural diversity, individual autonomy and legal responsibilities.Actively participates in multidisciplinary team approach to case management.Utilizes quality improvement tools to develop processes that promote continuous improvement in delivery of patient care, patient outcomes, or workflows.May participate in peer review and quality assurance

Job RequirementsMD, DO, or foreign equivalency training requiredBE/BC with ABMS or AOA requiredActive, non-restricted medical license in MN (or ability to obtain)Ability to obtain and/or maintain MN DEA certification and National Provider Identifier (NPI)Ability to meet criteria/qualifications for Credentialing and hospital privileges, if applicableACLS-BLS Tier 3 - Multisource required within 180 Days
