The Long-Term Acute Care Registered Nurse provides planningand delivery of direct and indirect patient care to medically complex patientsin long-term acute settings.

Essential Duties and Responsibilities:

Provides planning and delivery of direct and indirectpatient careResponsible for providing care to medically complex patientsSupervises care, treatment, and services of the patient(s)Documents the patient’s plan of care using identifiednursing diagnosis, expected patient outcomes and selected nursing interventionsMaintains confidentiality at all times with patients,medical staff and co-workersProvides a safe, comfortable and therapeutic patientenvironmentAdheres to and supports thepolicies/procedures/goals/objectives of the hospital in an attempt to providequality patient careRevises plan of care according to evaluation, changes inplan of care and nursing interventionsPerforms interventions according to identified priorities,plan of care, and facility policies and patient care outcome standardsIdentifies and prioritizes actions based upon patient carerequirementsMinimum Requirements:

Current RN license for the state in which the nursepractices inOne (1) year nursing experience in a long-term acute carefacility preferredExperience in acute rehabilitation for vents, wound care,cardiac, and stable post trauma patientsComplies with all relevant professional standards ofpracticeParticipation and completion of Amergis’ Competency programwhen applicableCurrent CPR if applicableTB questionnaire, PPD or chest x-ray if applicableCurrent Health certificate (per contract or stateregulation)Must meet all federal, state and local requirementsSuccessful completion of new hire training as applicable tojob siteUnderstand patient confidentiality and HIPAArequirementsAbility to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is requiredComputer proficiency requiredMust be at least 18 years of ageBenefits
