1. Reviews, verifies, and processes medication orders using the electronic health record to ensure that the medication is appropriate in terms of dose, dosage form, route of administration, frequency, duration of therapy, and patient's disease state in a timely manner. 2. Provides drug information services by responding to questions regarding medication compatibilities, adverse drug events, regimens, dosing, interactions, storage and disposal, compounding, therapeutic uses of medications and pharmaceutical supplies in a timely manner. 3. Actively participates in the development, maintenance, and implementation of hospital and department policy and procedures related to regulatory requirements for medication use, preparation, storage, restrictions, and administration. 4. Sustains and implements quality and safety initiatives and practices in the department and throughout the hospital. Reports adverse drug events and medication variances using the hospital variance reporting program (QSRS). 5. Documents pharmaceutical interventions according to departmental standards. 6. Performs all recordkeeping functions required by the Colorado State Board of Pharmacy rules and regulations regarding order interpretation, medication preparation and dispensing. 7. Oversees daily adherence to standard operational procedures and policies as required by CHCO, USP Chapters 797, 795 & 800 and Colorado State Board of Pharmacy rules & regulations. 8. Actively maintains competency, understanding and oversight of the fundamental practices and precautions for preparing and dispensing high quality hazardous drugs to prevent harm to patients, minimize exposure to personnel and minimize contamination of the work and patient-care environment. 9. Performs final evaluation for accuracy and appropriateness of doses dispensed, compounded and packaged. This includes sterile and non-sterile products prepared in pharmacy and pharmacy satellites. 10. Oversees and supervises activities of technicians, students and interns making sterile and non-sterile preparations and preparing or dispensing doses. Ensures that pharmacy products of the highest quality and accuracy are prepared, dispensed and delivered to the patient or storage area. 11. May precept pharmacy residents and students.
