A DAY IN THE LIFE OF A PHYSICAL THERAPIST

This position evaluates, plans, and administers medically prescribed or educationally relevant physical therapy treatment to clients in their homes or students in their respective educational environment. Responsibilities include but are not limited to:

Essential functions:

Performs a comprehensive PT assessments of assigned patients including, but not limited to, psychosocial, educational, safety and risk factor parameters.

Develops a plan of care to promote optimal functional independence and safety as documented by clinical record. Creates patient specific goals and ensures that the plan of care reflects logical interventions and advancement towards these goals.

Plans for and provides skilled physical therapy treatment including, but not limited to, therapeutic techniques, patient/care giver education, etc., assessing the patient's status and progress on each visit and ensuring quality of care as documented.

As Care Manager and/or team member, coordinates patient care with other clinical team members, including but not limited to, physicians, nurses, care givers, other rehab services, and students, ensuring optimal patient care and communication as noted.

Accurately and completely performs functions as related to school-based services, including, but not limited to, evaluations, documentation, development of Individual Educational Plan IEP's, participation in meetings, and provision of direct and consultative services. Fills out and completes insurance paperwork in a timely manner and in accordance with applicable regulations.

Supervises and directs clinical activities of PTAs, PT students, and home care aides.

Completes required paperwork in a timely manner.

Participates in in-service training programs for our customers including, but not limited to Rehabilitation, Medical, Nursing staff, volunteers, community groups, etc.

Proactively engages in the marketing and promotion of Rehab services including, but not limited to, developing specialized areas of service, participating in public health fairs, medical community, etc.

Works collaboratively with QI staff to maximize accuracy of clinical notes and OASIS documents.

Coordinates safe patient discharges and/or transfers (or upon knowledge of) and ensures all appropriate paperwork and patient information.

SKILLS, EXPERIENCE, AND QUALIFICATIONS:

Successful completion of an accredited Physical Therapy program required.

Two years of clinical Physical Therapy experience preferred. Prior Home Care experience preferred

Have self-direction and initiative, as well as be able to work collaboratively with colleagues within the agency.

Experienced with electronic medical records required.

Valid Vermont Driver's License and car insurance.
