Job Description:

AcclaimMulti-Specialty Group and JPS Health Networks Department of Family Medicine Department have an openingfor a full-time Advanced Practice Provider in the Polytechnic Medical Home.

This is a great opportunity to treat complexmedical issues, and to provide compassionate, quality care to those patientsthat need it most.Schedule: Monday-FridayJob Summary:The Advanced Practice Provider practices under the directsupervision of the supervising physician and is responsible for assisting withall aspects of patient care, including diagnosis, treatments and consultationswithin the inpatient and/or outpatient setting. This position will also educate patients about preventative care andprescribed treatments; may also conduct physicals, order tests and serve as apatient's healthcare provider.Duties and Essential Job Functions:Collaborates with the physician in overseeing and coordinating the multidisciplinary care delivered to all patients, to optimize quality patient care.Reviews patient medical charts to assist physician with diagnosis, treatment and care management.Makes appropriate patient referrals as necessary.Monitors guidelines for the management of care and for ensuring that the services are delivered in compliance with the guidelines established by JPS Health Network and Acclaim Physician Group.Provides care education to patients and families.Provides evidence-based and compassionate care to all patients as assigned to improve patient outcomes and increase patient satisfaction scores.Maintains a collaborative environment with providers, clinical staff, and support staff.Supports academic, educational and research endeavors.Attends Performance Improvement Programs and Peer Review committee meetings; and is an active participant in the process.Performs other job related duties as assigned.

Qualifications:

Required Education and Experience:Masters Degree in Nursing from an accredited school of nursing ORGraduate of an accredited Physician Assistant ProgramMinimum of 2 years working independently as an advanced practice provider in a primary care settingRequired Licensure/Certification/Specialized Training:Must have current Texas Physician Assistant license or Texas APRN license with FNP certification

Valid, unrestricted DEABasic Life Support (BLS) certification
