Job Description
The PPS Rehab Coordinator coordinates, monitors and submits the necessary documentation required by the State and Federal Government, facilitates IDT care plan meetings, works closely with commercial payers for pre certifications and case management, responsible for utilization review, monitors and conducts staff training on the IRF-PAI, monitors patient charts for correct FIM scoring, ensures that the program meets CMS compliance requirements and encourages sensitivity to cultural diversities and promotes understanding of the aging population.
Responsibilities
Monitors facility wide PPS forms for accuracy and completion within the appropriate time frames and reviews and maintains assessment protocols
Responsible for clinical data collection, entry, transmission and analysis to ensure compliance
Acts as a resource person for nursing and therapy staff in the IDT meetings and ensures an appropriate plan of care for each patient
Ensures confidentiality of all resident information. Respects the rights, privacy and dignity of each patient
Maintains clinical competencies by utilizing the nursing process or frame of reference for the delegation and coordination of patient care within the scope of nursing practice
Maintains integrity of electronic submission and works with MIS to update software. Ensures electronic submission of assessments to state per schedule
Acts as a resource person for nursing and other disciplines to ensure compliance in data collection and completion of care plans
Coordinates data input with coding office, billing office and admissions to ensure validity
Keeps abreast of all Federal Government Changes related to PPS and incorporate regulatory mandates into routine practices
Identifies quality assessment/data problems and initiates and implements outcome measures
Understands other certifying agency standards such as JCAHO and CARF and ensures that data is accurate and complete to meet specific requirements
Works with Social Worker, AR Director and Medical Director to coordinate IDT Care Plan Meetings with the prescribed time constraints
Reviews and enforces with staff the need for consistent, clear, and accurate documentation in the medical record to support outcomes
Updates team and medical staff with new regulations related to assessments and reimbursement
Qualifications
Required Education: Graduate of an accredited School of Nursing or a licensed Physical, Occupational, or Speech Therapist, PTA, COTA.
Preferred Education: Bachelor's degree
Required Experience: At least 2 years if Acute Inpatient Rehabilitation experience. Computer knowledge
Required Certifications and Licensures: Licensed or eligible for licensure in the state of Virginia by the State Board of Nursing or Rehab Licensing Board
