Since our inception, SummitWest Care has served several thousand clients and families in Mesa County and we have had the support and donations from organizations such as St. Mary's Hospital, Rocky Mountain Health Plans and Hilltop. Because of our strong staff expertise and leadership and the profound belief that people deserve to heal at home, we have received several awards for quality and excellence in home health care service. Our clinicians have a long history of collaboration with area physicians to help their clients get well after premature birth, complicated orthopedic surgeries and chronic illnesses. We value education and staying on the cutting edge of procedures and techniques to support our clinicians to be aware and responsive to health care needs.
Your day to day as a Licensed Physical Therapist AssistantAs a Licensed Physical Therapist Assistant at SummitWest Care, you will work with clients in their homes to build and maintain independence while ensuring they receive the highest level of care. Your role will involve providing timely and accurate documentation to keep track of each client's progress, allowing for seamless communication and collaboration with our clinical team to tailor the treatment plan according to individual needs. Additionally, you will educate clients and their families in alignment with the physical therapy care plan, emphasizing empathy and support throughout their recovery journey. This collaborative approach not only enhances client outcomes but also fosters a flexible and inclusive environment where both clients and caregivers thrive.
What matters mostTo excel as a Licensed Physical Therapist Assistant at SummitWest Care, you must possess a valid Active CO PTA License, a Valid Driver's License, and Current Auto Insurance to ensure safe transportation to client locations. Strong organizational and time management skills are essential for providing timely and accurate documentation of each client's progress. Effective communication abilities will enable you to collaborate seamlessly with the clinical team, promoting a supportive and inclusive atmosphere. Additionally, empathy and problem-solving skills are crucial for educating clients and their families according to the physical therapy care plan.
